Job Description

Experienced and compassionate Registered Nurses with a background in home health are invited to take on a rewarding assignment providing skilled care to patients in a field-based setting. In this contract position, youll play a pivotal role in ensuring patients receive individualized, high-quality nursing services in the comfort of their homes.
Schedule:
Monday to Friday, 8:00 AM 5:00 PM Key responsibilities include: Conducting OASIS assessments and developing comprehensive, patient-centered care plans Delivering skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management during 35 daily patient visits (2532 points per week) Monitoring and evaluating patient progress, promptly communicating changes to the healthcare team and physicians Educating patients and families on disease processes, medications, treatments, and preventive care Documenting activities and assessments in Home Care Home Base (HCHB) system Collaborating closely with physical therapists, occupational therapists, home health aides, and medical social workers for coordinated patient care Engaging in case conferences, staff meetings, and professional development opportunities Adhering to all regulatory and agency standards for compliance and quality care
Qualifications:
Current RN license in good standing At least one year of recent home health experience (field-based preferred) Proficiency with Home Care Home Base (HCHB) and OASIS assessments Reliable transportation, valid drivers license, and current auto insurance Strong assessment, time management, and organizational skills Ability to work independently in the field BSN and familiarity with Medicare/home health compliance preferred Benefits include weekly pay, comprehensive medical, dental, and vision insurance options, 401(k) plan options, round-the-clock support, mileage reimbursement, and possibilities for contract renewal.
