Job Description

Arcadia New England is immediately hiring a Registered Nurse field supervisor to conduct intake/opens and quarterly field supervisory visits of personal care staff and their clients in the home on a per diem basis. Flexible schedules, non-facility based. Per diem, work as little or as much as you like. The RN is responsible for opening and intakes based on an authorized care plan review with the client in their home, and performing quarterly in person observed performance and client supervisory visits for home care field employees and patients to ensure staff best practice and patient health and wellness as indicated by the individual plan of care. This is a field-based position, not facility based, where the RN Field Supervisor provides professional nursing oversight to the client and caregiver in the home as prescribed by the plan of care and in compliance with regulations as established by the Massachusetts State Board of Nursing, and the policies of the company and applicable referral sources.
KEY RESPONSIBILITIES AND DUTIES
Open and Intake of care plans and patient case loads, including medication management visits Performs observed performance and supervisory visits with caregiver field staff and clients to ensure proper skills and care-based competencies in accordance with the plan of care, including evaluating care plans, performing observations and patient health and welfare supervisory assessments. Observes the client for changes in health condition and reports these changes to the office staff. Provides instructions to caregivers on disease process, medication, diet, safety and other instructions as ordered by the plan of care and recognizes and utilizes opportunities for health counseling with clients and families/caregivers. Supports caregivers by communicating caregiver needs to office staff.
QUALIFICATIONS
Graduate of an accredited school of professional nursing. Currently a licensed RN through the Massachusetts State Board of Nursing. At least one (1) year's general nursing experience in a clinical area, home care supervisory or assessment experience. Valid Driver's license and access to car transportation.
