Home Health Admissions RN | Three-Day Full-Time Schedule Saturday-Monday | Newburyport to Haverhill $88,000-$105,000, based on experience | Up to $5,000 sign-on bonus Three days. Full-time benefits. A chance to help patients start care at home. The first home health visit sets the tone for everything that follows. At Elara Caring, we're looking for an experienced RN who can walk into a patient's home, see the full picture, and build a thoughtful plan for what comes next. As an Admissions RN , you'll focus on start-of-care visits across Greater Boston and the North Shore. You'll have the independence to use your clinical judgment, with a team behind you to help patients move smoothly into ongoing care. What your work will look like Complete start-of-care visits and comprehensive assessments Evaluate clinical needs, medications, home safety, and caregiver support Create individualized plans of care with physicians and the clinical team Educate patients and families so they understand the plan and feel prepared Document admissions accurately and communicate key findings to the care team What you'll bring An active, unrestricted Massachusetts RN license At least one year of nursing experience Home health experience is required ; admissions or start-of-care experience is preferred Strong assessment, documentation, and independent decision-making skills A valid driver's license, reliable transportation, and current auto insurance Experience with HomeCare HomeBase (HCHB) is a plus. An associate degree in nursing is required; a BSN is preferred.
