A Day in the Role As a Home Health Registered Nurse , you will: Complete initial and ongoing nursing assessments in patients' homes Manage an assigned caseload and coordinate each patient's care Develop, implement, and update individualized Plans of Care Provide skilled nursing treatments in accordance with physician orders Monitor patients' conditions, medications, symptoms, and responses to treatment Identify and promptly communicate changes in condition or safety concerns Educate patients and caregivers about medications, treatments, diagnoses, and self-care Coordinate services with physicians, therapists, social workers, aides, and other care partners Help patients and families access appropriate clinical and community resources Take proactive steps to reduce avoidable hospitalizations and emergency department visits Maintain accurate, timely, and complete clinical documentation Follow all clinical, safety, regulatory, and HIPAA requirements You're a Great Fit If You Have A degree or diploma from an accredited nursing program A current, unrestricted Registered Nurse license in the applicable state At least one year of nursing experience in a clinical care setting Strong assessment, communication, organization, and care-coordination skills The confidence to work independently while remaining connected to a clinical team A dependable vehicle, valid driver's license, and current automobile insurance The ability and willingness to travel throughout the assigned territory
