Job Summary Brookside Rehab & Healthcare Center is a small independently owned facility. We are seeking a fulltime MDS Coordinator to join our team The MDS Coordinator is responsible for assuring timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. The MDS Coordinator includes but is not limited to; MDS accuracy, MDS transmission, care plans, UR, nurse note accuracy, discharge planning, QAPI, family meetings, policies and procedures, federal and state compliance. The MDS Coordinator monitors completion of MDSs by other disciplines with timeframes specified by guidelines. Sign on Bonus, PIB option ( Pay instead of Benefits)

Required:

Must be a Nurse in good standing and currently licensed by the State of Massachusetts.. Previous experience in a hospital, long-term facility, or other healthcare related facility is required Prior experience completing MDSs and Care Plans is required Familiar with Point Click Care (PCC) Join our team to play a crucial role in delivering exceptional resident care through precise documentation and regulatory compliance!

Pay:

$41.00 - $46.00 per hour Expected hours: 40.0 per week
