Responsibilities:
Develop and manage comprehensive, individualized care plans, documenting interventions and measurable outcomes. Continuously evaluate patient progress, adapting care plans as needed. Educate patients and caregivers, promoting independence and self-management. Perform skilled nursing procedures using standard homecare equipment and supplies. Collaborate with interdisciplinary teams, participating in regular case conferences. Engage patients and families in goal setting and care plan revisions. Maintain accurate clinical records and reports. Communicate effectively with physicians, healthcare teams, and families.
Qualifications:
Passionate about providing exceptional home healthcare. 1-2 years of recent RN experience (ICU, CCU, Med-Surg, Oncology, Gerontology). Active RN license in Maryland. Current CPR certification. Proficiency in IV therapy and wound care. Strong organizational and communication skills. Basic computer proficiency (Homecare Homebase EMR experience a plus). Valid driver's license, reliable transportation, and an acceptable driving record.
Schedule:
Monday•Friday; 8:00am-4:30pm (No On-Call!) with occasional weekend rotation
Compensation:
$89,856•$125,000 per year Mileage reimbursement at the IRS rate
