Lions Rehab Center is a 101-bed skilled nursing facility located in Cumberland, Maryland. We are committed to providing a warm and welcoming environment where our residents and patients feel comfortable, safe, and cared for.
Job Summary:
As our MDS RN, you will be responsible for conducting Minimum Data Set (MDS) assessments and ensuring that each patient's care plan is comprehensive and up-to-date. You will work closely with our interdisciplinary care team to develop and implement individualized care plans that meet the unique needs of each patient. Your attention to detail, excellent communication skills, and ability to work independently will make you an invaluable member of our team.
Responsibilities:
Conduct MDS assessments and develop comprehensive care plans Collaborate with the care team to ensure accuracy and completeness of patient records Communicate with physicians, families, and other healthcare professionals to ensure seamless care Maintain accurate and up-to-date patient records Participate in quality improvement initiatives to improve patient outcomes
Requirements:
1 year of experience Strong understanding of MDS regulations and guidelines Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong attention to detail and organizational skills
