• Eligible candidates are hired (offer accepted) into a Full or Part time RN position. Bonus amount prorated for Part time hires, per diem hires are ineligible.

• Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 6 months separation from employment.

Summary:

This is a 40-hour a week, Day shift position with multiple shift-start options.

Unit Bio:

As a Centralized Post-Acute Bed Placement Coordinator , you'll play a pivotal role in helping patients transition safely and successfully to the next level of care. Working across our health system, you'll partner with care teams, patients, families, and community providers to ensure patients receive the right care, in the right setting, at the right time. This is an ideal opportunity for a clinically experienced professional who enjoys problem-solving, care coordination, and collaboration across the healthcare continuum. Your expertise will directly impact patient outcomes, reduce delays in care, and help individuals navigate important healthcare decisions during critical moments in their recovery journey. What You'll Do Coordinate placements for patients transitioning from acute care to post-acute settings, including skilled nursing facilities, inpatient rehabilitation, long-term acute care, home health, hospice, and other community-based services. Serve as a clinical resource for determining the most appropriate level of post-acute care based on patient needs, goals, and payer requirements. Collaborate closely with hospital teams, post-acute providers, patients, and families to facilitate timely, seamless care transitions. Review clinical information and assess care needs to support placement decisions and referral processes. Build and maintain strong relationships with post-acute care partners across the region. Help improve patient flow, reduce barriers to discharge, and ensure patients receive high-quality care throughout their recovery journey. Required Minimum Knowledge, Skills, and Abilities (KSAs)

Education:

Associate's degree in appropriate clinical field required; bachelor's degree preferred.

License/Certifications:

Current licensure as a Registered Nurse, Physical Therapist or Occupational Therapist in the State of Maine required. Current Basic Life Support (BLS) certification is required or must be obtained within 30 days of start date. Case Management certification (CCM, ACM-RN, or equivalent) preferred.

Experience:

Three years' direct clinical experience required. Experience working in a skilled nursing facility or post-acute entity, discharge planning, utilization management, admission management, or post-acute care coordination preferred. Experience in a healthcare setting involving care transitions across multiple levels of care preferred.

Additional Skills/Requirements Required:

Knowledge of post-acute levels of care, including skilled nursing, inpatient rehabilitation, long-term acute care, home health, hospice, and related services. Knowledge of healthcare reimbursement and coverage requirements affecting post-acute services. Ability to review and interpret clinical documentation and patient care needs. Proficiency with electronic medical records and referral management systems.

Additional Skills/Requirements Preferred:

Knowledge of value-based care models and population health strategies. Knowledge of post-acute provider networks and community resources. The base range is an estimate and does not reflect the full value of our total compensation package. The pay range for this position is $83,324.80 to $112,736.00 per year. The pay range for this position for a Physical Therapist is $83,324.80 to $112,736.00 per year. The pay range for this position for an Occupational Therapist is $77,147.20 to $104,353.60 per year. Compensation for part-time positions is determined on a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
