Seeking an experienced Nurse Practitioner with a passion for home health to conduct comprehensive risk assessments for Medicare Advantage members across Androscoggin County. This vital per diem role focuses on meaningful, one-on-one patient interactions to ensure care continuity and support the health needs of individuals in their residences, including those in Lewiston as well as neighboring towns such as Auburn, Lisbon, and Poland.

Primary Responsibilities:

Complete annual and post-discharge home health risk assessments (average 45 minutes per patient) Review past and present medical and medication histories Educate patients on condition management and their medications during in-home visits Identify care gaps and advise on appropriate next stepsno need to prescribe medication or develop treatment plans Support care management for older adults transitioning post-discharge or undergoing annual reviews

Qualifications:

Nurse Practitioner license valid in Maine Board Certification (AANP or ANCC) Current CPR/BLS certification MSN from an accredited nursing program At least 6 months of recent experience as a Nurse Practitioner preferred This opportunity offers compensation of $80-$90 per evaluation, with a mileage stipend and compensation for administrative time, ensuring your professional efforts are valued and rewarded.
