As a Nurse Practitioner specializing in home health, you will make a difference by performing comprehensive home health risk assessments for Medicare Advantage plan members across Cumberland County. This per diem role offers flexibility and the opportunity to serve patients in Portland, South Portland, Westbrook, and surrounding areas, with each assessment visit lasting around 45 minutes. Responsibilities vary from annual health assessments to post-discharge visits, focusing on evaluation and education rather than direct treatment or prescribing.

Key Responsibilities:

Conduct thorough annual home health risk assessments in member homes Review and evaluate patient medical and medication histories Educate patients on medical conditions and proper use of medications Detect gaps in care and recommend appropriate actions to address them No responsibility for developing formal treatment plans or prescribing medications

Qualifications:

Valid Nurse Practitioner license in Maine Master of Science in Nursing (MSN) from an accredited program Board Certification through AANP or ANCC Current BLS/CPR certification At least six months of recent Nurse Practitioner experience preferred
