None Requirements Minimum 1 year of Home Health/Home Care RN experience required Active Michigan RN license required Active AHA BLS certification required Active driver's license required Reliable vehicle and active auto insurance required Clean DMV record required Experience with IVs, wound VACs, and central lines highly preferred 2 supervisory references from the last 2 years Current Home Health skills checklist required Permanent address required Candidates within 50 miles of the facility are not accepted MI LTC fingerprinting results required before starting Epic experience preferred Responsibilities Assess clients' health needs and determine appropriate nursing care requirements Develop and implement individualized plans of care Coordinate and direct care provided by other nursing staff Evaluate the effectiveness of care plans and modify interventions as needed Provide skilled nursing care in the home setting Monitor patient conditions and identify changes requiring intervention Provide nursing care across neonatal, pediatric, adolescent, adult, and geriatric populations as applicable Perform and manage IV therapy, wound VAC care, and central line care Educate patients and families regarding treatment plans, medications, and ongoing care Maintain accurate clinical documentation and follow established nursing standards Application Question(s): Clean DMV record required
