Job Description
Registered Nurse (RN), Supports Coordinator Region II Commission on Services to the Aging Union Position. No Nights or Weekends.
Full Time:
40 hours per week. Work schedule M-F excluding observed holidays. Benefits include paid time off and paid holidays, health insurance and employer contribution to a 403(b) retirement account. Essential Duties and Responsibilities Conduct comprehensive in-home participant assessments. Utilize nursing skills to gather and analyze medical data from initial assessments, including medication and disease management. Work with family members and other volunteer caregivers to maximize available services to the participant. Develop a plan of care identifying needed services, supports, and interventions. Arrange and coordinate delivery of needed services. Monitor participant plans of care to assure delivery of needed services. Evaluate and modify plans of care as required. Establish and maintain liaison with nursing home administrators, hospital discharge planners, human services providers, home care agency staff, and other community resources. Assist other care managers with questions resulting from reassessments and other situations in medically complex cases. Work with special medically complex cases requiring a special memorandum of understanding with the Michigan Department of Community Health. Participate in regular staff meeting and training events. Participate in community events to teach, assist and advocate. Perform other duties as assigned. Qualifications Must be a Registered Nurse licensed in the state of Michigan with at least one year of community nursing experience. At least one year experience in the field of aging. Able to analyze varied environmental factors in relation to participants' well being. Working knowledge of health related service delivery systems. Good interview and telephone skills, as well as the ability to deal with older adults in a caring and helpful (assertive and tactful) manner. Good communication and relationship skills. Strong computer fundamentals and proficiency with Microsoft Office products. Ability to travel within Lenawee, Hillsdale and Jackson Counties.
