We are seeking a compassionate, organized, and tech-comfortable licensed nurse to provide client assessments, case coordination, medication management, and support services for older adults throughout Wexford County. The position is part-time, 16-24 hours per week. The position offers a very attractive and unique opportunity for flexible scheduling, with the selected candidate having input into their schedule based on program and client needs. The Nurse Care Coordinator / Assessment Nurse plays a key role in helping seniors remain safe, supported, and well cared for in their homes. This role does not involve routine hands-on personal care.

It is primarily focused on:

in-home initial assessments, re-assessments, monitoring client needs, completing required documentation, and supporting referrals. The ideal candidate is dependable, detail-oriented, comfortable working independently in the community, and able to communicate clearly with clients, caregivers, providers, and internal team members. Key Responsibilities:

• Travel to client homes in Wexford County and conduct initial and re-assessments to identify care needs, risks, strengths, preferences, and appropriate support services
• Provide care coordination, case follow-up, and support services for clients and caregivers
• Accurately complete client paperwork, charting, service documentation, and required follow-up in a timely manner

Requirements:

Current Michigan RN or LPN license Reliable transportation and valid driver's license Willingness to travel across all of Wexford County Ability to pass background check and TB screening Comfortable learning and using technology Strong assessment, documentation, organization, follow-through, and communication skills Ability to work independently, manage time effectively, and use sound professional judgment in client homes and community settings Ability to pass required background checks, screenings, and other employment requirements Why Join Us?
