In this role, you will provide high-quality, patient-centered care in home health settings, helping patients navigate their recovery and health management plans comfortably.

No Key Responsibilities Provide professional nursing care, assessments, and education to patients in their homes. Develop, implement, and evaluate individualized patient care plans in collaboration with physicians. Monitor patients' conditions and report any critical changes. Document all care accurately and timely using agency guidelines. Qualifications & Requirements Active Registered Nurse (RN) license in the state of Michigan. Current BLS certification. Reliable transportation, valid driver's license, and auto insurance (home health travel required). Ability to complete standard background checks, health screenings, and credentialing (including MMR, Varicella, TB test, physical exam, and drug screen). CA02

Pay:

$55.00 - $65.00 per hour Expected hours: 40.0 per week
