• Grand Rapids, MI Pay:

• 70/visit , based on Medicare home health and OASIS experience Community Care Givers is seeking an experienced Registered Nurse (RN) to provide Medicare-certified home health services and complete Start-of-Care (SOC) admissions and OASIS assessments throughout the Grand Rapids area.

Responsibilities Complete SOC, ROC, recertification, transfer and discharge visits as needed Perform comprehensive nursing assessments Develop and update individualized Plans of Care Complete medication reconciliation and patient education Coordinate physician orders and communicate with the care team Assess skilled nursing and homebound needs Maintain accurate documentation to support Medicare eligibility and medical necessity Qualifications Current Michigan RN license in good standing At least 1 year of RN experience Medicare home health/OASIS experience strongly preferred Experience with Medicare SOC admissions preferred Strong clinical assessment, documentation and communication skills Reliable transportation and valid driver's license Comfortable using electronic medical records and mobile documentation Schedule Monday-Friday, primarily daytime hours Some flexibility may be available based on experience and agency needs.
