Collaborate with interdisciplinary teams including nursing, therapy, social work, and physicians to gather pertinent information for comprehensive case management. Participate in clinical documentation improvement initiatives to enhance the quality of medical records and support compliance with NCQA standards. Requirements Valid Registered Nurse (RN) or (LPN) license with current state licensure. Proven experience with MDS assessments in long-term care or skilled nursing facilities is highly desirable. coding, CPT coding, DRG assignment, and Medicare regulations. Familiarity with EMR/EHR, Pointclick care proficiency in medical documentation review is essential. Excellent understanding of physiology, anatomy, medical terminology, and healthcare regulations including HIPAA compliance. Ability to perform detailed documentation review while maintaining attention to detail under tight deadlines. Strong case management skills with a focus on discharge planning, hospice care coordination, and outpatient services are advantageous.
