Coverage Areas:
Houghton, Keweenaw (partial) and Baraga (partial) counties
Shift:
M-F 8 am - 4:30 pm
On Call/Weekend Requirement:
Once a month weekends and 3 weekdays a month
Roles & Responsibilities:
Provide skilled nursing services in accordance with physician orders, agency policies, and professional nursing standards.

Perform comprehensive patient assessments and ongoing evaluations.

Monitor patient conditions and identify changes requiring intervention or physician notification.

Deliver individualized patient care that promotes safety, recovery, and independence in the home environment.

Manage an assigned home health patient caseload.

Develop, implement, and update individualized plans of care in collaboration with physicians and interdisciplinary team members.

Complete Start of Care (SOC) assessments and admissions.

Perform recertification assessments and ongoing evaluation visits.

Conduct discharge planning and discharge visits.
Minimum Qualifications:
Active and unrestricted Registered Nurse (RN) LicensePrevious Home Health Nursing ExperienceValid driver's license and reliable transportationCurrent automobile insurance

PICC Line care and managementWound VAC therapyComplex wound careStrong clinical documentation skills
