Primary Purpose The Clinical Care Registered Nurse (CCRN) role provides direct nursing care to participants. This role works independently in the application of advanced nursing knowledge and skills and can manage complex clinical situations. This role collaborates with both licensed and non-licensed personnel. Accountability Chart Key Roles Triaging emergent situations and able to remain independent in the application of advanced nursing knowledge. Monitoring acute and chronic medical conditions in coordination with Primary Care Providers (PCPs). Utilizing care plans independently to provide clinic and home-based direct care. Administers treatments, procedures and medications as prescribed to participants. Duties and Responsibilities Routine monitoring of acute and chronic medical conditions following the participants' goals of care. Performs wound care and treatments as prescribed in the participant's plan of care. Administers medications, immunizations, and other care as prescribed. Performs assessments and actively participates in care plans. Makes home visits regularly to provide care and treatment as ordered by the PCP. This may include wound care, lab draws, nurse assessments, and other nursing tasks. Performs on-call needs and determines immediate needs for the participant as needed. Works beyond normal working hours and on weekends and holidays, as necessary. Must complete all required education and employee file requirements on time. Documents in the participant's medical record as required by policy. Must care, support, and collaborate with everyone involved in the participant's care, including staff and the participant's family, using the participant-centered care model. Keeps the leader informed of problems or issues. Performs other duties as assigned. Required Qualifications Bachelor's of Science in Nursing preferred; Associate degree required. RN-Nursing Diploma or Degree from an accredited School of Nursing or University. Proficiency with Microsoft Word and Outlook. Well-developed ability to show discretion and maintain confidentiality. Strong communication skills, both written and verbal. Minimum of one year of experience working with the frail, elderly, or long-term care population. Minimum of one year home care experience preferred. Minimum of one-year wound care experience preferred. Specific Requirements Demonstrates the core values of belonging, ownership, partnership, and excellence. Current AHA CPR certification, or willingness and ability to obtain certification through Senior Care Partners, PACE. Ability to practice within the appropriate scope of authority. Ability to successfully complete position-specific competencies established by Senior Care Partners, PACE and approved by CMS before working independently. Knowledge of current medical practices, procedures, and applicable long-term care laws, regulations, and guidelines. Ability to make sound independent clinical decisions when circumstances require. Strong interpersonal skills and the ability to communicate respectfully with participants, families, staff, government representatives, visitors, and community partners. Ability to plan, organize, implement, and support organizational goals, policies, procedures, and quality-care initiatives. Patience, tact, enthusiasm, and a positive disposition when working with participants and families in challenging circumstances. Must be 18 years of age or older.
