Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. You're the Clinical Leader for Your Patients As an RN Case Manager, you're not simply completing scheduled visits. You're connecting the pieces. You'll understand each patient's clinical picture, monitor changes over time, communicate with providers, coordinate the interdisciplinary team, educate caregivers, and advocate for what your patient needs. When something changes, you're the nurse who knows the patient . What You'll Do Manage an assigned hospice caseload throughout the Greater Twin Cities Metro Area. Perform comprehensive nursing assessments and develop individualized plans of care. Manage pain, symptoms, medications, wounds, and changes in condition . Coordinate care with physicians, hospice aides, social workers, chaplains, therapists, and other clinical professionals. Educate patients and families about medications, symptoms, disease progression, and comfort measures. Collaborate with assisted living, memory care, skilled nursing, and other community partners. Help coordinate smooth transitions into hospice care. Maintain accurate and timely clinical documentation. Advocate for each patient's comfort, dignity, and goals of care . Bring Your Experience. We'll Help You Apply It to Hospice. Hospice experience is preferred, but strong nurses come from many different clinical environments. We're interested in RNs with backgrounds in: Hospice or Palliative Care. Home Health. Oncology. ICU or Critical Care. Medical-Surgical Nursing. Long-Term Care. Skilled Nursing. What We're Looking For Active Minnesota Registered Nurse (RN) license . Strong clinical assessment and critical thinking skills. Ability to independently manage and prioritize a patient caseload. Excellent patient and family communication. Strong care coordination and organizational skills. Compassionate, dependable approach to patient care. Valid driver's license and reliable transportation.
