We're hiring an experienced Registered Nurse to manage home health patients throughout the Northwest Metro and north toward St. Cloud , with coverage including Plymouth, Buffalo, Monticello, St. Cloud, and surrounding communities. You'll combine hands-on nursing with clinical coordination, patient education, physician communication, and interdisciplinary collaboration while earning $85,000-$130,000+ annually plus a $6,000 First-Year Bonus. This Is More Than Completing the Visit As an RN Case Manager, you're not responsible for one isolated piece of someone's care. You're helping make sure all the pieces work together. You'll assess your patients, understand their diagnoses and medications, communicate with physicians, collaborate with therapy, educate caregivers, update the plan of care, recognize changes, and make sure important information doesn't get lost between everyone involved. Your job is to understand how it all fits together. You'll work directly with physicians, PTs, OTs, SLPs, social workers, patients, caregivers, and other clinical professionals to keep the plan moving in the same direction. When something changes, you're positioned to recognize it. When a provider needs an update, you communicate it. When a family doesn't understand the plan, you educate them. When another discipline identifies a concern, you help coordinate what happens next. You're not just participating in the care plan. You're helping drive it. What You'll Do Manage an assigned home health caseload Complete comprehensive nursing assessments Perform admissions, recertifications, and ongoing skilled visits Develop and update individualized plans of care Complete OASIS assessments and documentation Provide medication management and disease education Manage wounds and other skilled nursing needs Monitor medically complex and chronic conditions Recognize changes in condition and coordinate appropriate interventions Educate patients and caregivers Coordinate with physicians and the interdisciplinary care team Help reduce avoidable emergency room visits and rehospitalizations Participate in an on-call and weekend rotation Your Nursing Experience Has Prepared You for More Maybe Med/Surg taught you to prioritize six things happening at once. ICU strengthened your assessment and critical-thinking skills. Transitional care taught you what can go wrong after discharge. Skilled nursing taught you how to coordinate across disciplines. Case management gives you a place to bring those skills together. We're interested in experienced nurses from Home Health, Med/Surg, Acute Care, ICU, Transitional Care, Skilled Nursing, Rehabilitation, Case Management, and other clinically complex settings . Home health and OASIS experience are preferred, but we're also interested in strong RNs ready to take the next step.
What You Bring Required:
Active Minnesota Registered Nurse (RN) license Three or more years of nursing experience Strong clinical assessment and critical-thinking skills Excellent patient and caregiver communication Ability to independently manage a productive patient caseload Strong organization and follow-through Valid driver's license and reliable transportation
Preferred:
Home health nursing experience OASIS experience Previous case management experience Coverage Area You'll support patients throughout the Northwest Metro and north toward St. Cloud, including areas around Plymouth, Hamel, Medina, Loretto, Corcoran, Rockford, Buffalo, Monticello, Maple Lake, Annandale, Kimball, Clearwater, Clear Lake, Becker, Big Lake, St. Cloud, and surrounding communities. Territory assignments and scheduling are designed around patient and clinical needs throughout the service area.
