Senior Care Solutions is seeking an experienced Registered Nurse (RN) MDS Coordinator to provide interim support to long-term care communities across Minnesota. This is a traveling position for an experienced MDS professional who enjoys stepping into new environments, supporting facility teams, and helping ensure accurate reimbursement and regulatory compliance. As an Interim MDS Coordinator, you will manage and support the RAI/MDS process while providing knowledgeable, hands-on assistance to our client communities. Responsibilities Coordinate and manage the complete MDS assessment process, ensuring accurate and timely completion and submission. Ensure compliance with CMS, federal, and state requirements related to MDS, RAI, care planning, and reimbursement. Review clinical documentation and MDS coding to support reimbursement accuracy and regulatory compliance. Apply knowledge of

PDPM, MDS

3.0, and current CMS guidelines. Collaborate with nursing leadership, interdisciplinary teams, therapy, and other facility staff. Identify documentation or process concerns and provide guidance to facility teams as appropriate. Assist communities during staffing transitions, vacancies, increased workload, or other temporary MDS needs. Maintain accurate records and documentation in accordance with regulatory and facility requirements. Qualifications Active Registered Nurse (RN) license in Minnesota required. Minimum of 3 years of MDS coordination experience in skilled nursing/long-term care. Strong working knowledge of the RAI process, MDS 3.0, PDPM, reimbursement, and CMS guidelines. Experience independently managing MDS assessments and deadlines. Strong organizational, analytical, and communication skills. Ability to quickly adapt to different facility environments, systems, and teams. Willingness and ability to travel to client communities as assignments require.
