At Superior Hospice, we're hiring an experienced RN Case Manager to provide care throughout the Northwestern Greater Twin Cities Metro Area . You notice the subtle change in breathing. You recognize that pain isn't being controlled as well as it was last week. You hear something in a caregiver's concern that tells you they need more support. Because you know your patients, you have the context to recognize those changes and act. As an RN Case Manager, you'll use your clinical judgment to anticipate needs, manage symptoms, coordinate care, and help patients remain comfortable wherever they call home . What You'll Do Manage an assigned hospice caseload throughout the Northwestern Greater Twin Cities Metro Area Perform comprehensive nursing assessments Develop and update individualized plans of care Manage pain, dyspnea, agitation, wounds, medications, and other symptoms Identify changes early and coordinate appropriate interventions Educate patients and caregivers about medications, disease progression, symptoms, and comfort measures Coordinate with physicians, pharmacies, and community providers Collaborate with hospice aides, social workers, chaplains, and the interdisciplinary team Work with Assisted Living, Memory Care, Skilled Nursing, and other community partners Maintain accurate and timely clinical documentation Advocate for each patient's comfort, dignity, and goals of care Your Experience Can Translate to Hospice Hospice experience is preferred, but we're also interested in experienced RNs from Home Health, Palliative Care, Oncology, ICU/Critical Care, Med/Surg, Skilled Nursing, Long-Term Care, Assisted Living, Memory Care, and Community-Based Nursing . If you've spent your career assessing complex patients, recognizing changes, educating families, coordinating with providers, and determining what needs to happen next, you already have skills that can make you successful in hospice. What We're Looking For Active Minnesota Registered Nurse (RN) license Strong clinical assessment and critical-thinking skills Ability to independently manage and prioritize a patient caseload Excellent patient and family communication Strong organization and care coordination Confidence working independently in the community Valid driver's license and reliable transportation Willingness to travel throughout the Northwestern Greater Twin Cities Metro Area Hospice experience preferred, but strong transferable nursing experience is encouraged Spend Less Time Chasing the Day.
