Ever Wonder What Happened to Your Patient After They Went Home? In the hospital, SNF, or transitional care setting, you help stabilize the patient. You educate them. You prepare them for discharge. Then they leave. Home health is where you get to see what happens next. Did they understand the medication changes? Can they safely manage at home? Is the wound improving? Did they actually schedule the follow-up appointment? Is the family handling everything as well as they thought they would? At Home Health Care, Inc., our Registered Nurse Case Managers help patients navigate that next chapter. You'll provide one-on-one nursing care in patients' homes throughout Coon Rapids, Blaine, and surrounding communities , with patient visits generally scheduled within approximately a 35-mile radius of your home address . A mobility plan can sound reasonable until you see the stairs they have to navigate. A caregiver may say they're comfortable helping until you're sitting across from them and realize they're overwhelmed. The home tells you things the chart can't. You'll use that information to assess your patient, provide skilled nursing care, update the plan of care, educate patients and caregivers, communicate with providers, and coordinate with the interdisciplinary team. What You'll Do Manage an assigned home health caseload Complete comprehensive nursing assessments Perform admissions, recertifications, and skilled visits Develop and update individualized plans of care Complete OASIS assessments and documentation Provide medication management and disease education Manage wounds and other skilled nursing needs Monitor medically complex and chronic conditions Recognize and respond to changes in condition Educate patients and caregivers Coordinate with physicians, PTs, OTs, SLPs, social workers, and other clinicians Help prevent avoidable emergency department visits and rehospitalizations Participate in an on-call and weekend rotation Your Nursing Background Can Translate We're interested in experienced RNs from Home Health, Med/Surg, Acute Care, ICU, Transitional Care, Skilled Nursing, Rehabilitation, Case Management, and other clinically complex settings . Maybe you've spent years preparing patients to leave your unit or facility. Now you can use those same assessment, education, prioritization, and critical-thinking skills to help them successfully stay home once they get there. Home health and OASIS experience are preferred, but we're also interested in strong RNs ready to make the transition. What You Bring Active Minnesota Registered Nurse (RN) license Three or more years of nursing experience Strong clinical assessment and critical-thinking skills Excellent patient and caregiver communication Ability to independently manage a productive caseload Strong organization and follow-through Valid driver's license and reliable transportation Home health, OASIS, or case management experience preferred Don't Just Prepare Someone to Go Home.
