This role involves assessing patient symptoms, obtaining relevant medical history, identifying urgent conditions, and determining appropriate patient dispositions. The nurse will also provide patient education, communicate treatment instructions, and accurately document all interactions in EPIC. Key Responsibilities Perform telephone and electronic message triage for patients of all ages with a variety of acute and chronic medical concerns. Assess patient symptoms, obtain relevant medical history, and identify urgent or emergent conditions requiring immediate medical attention. Determine appropriate disposition, including scheduling same-day or routine appointments, directing patients to urgent care or emergency services, or escalating concerns to a physician or advanced practice provider. Follow established clinical triage protocols, practice guidelines, and organizational policies when making patient care recommendations. Respond to patient calls, portal messages, and clinical inquiries in a timely, professional, and compassionate manner. Provide patient education regarding medications, treatments, symptom management, preventive care, and appropriate follow-up. Communicate provider recommendations and treatment instructions clearly to patients and caregivers. Document all patient interactions, assessments, recommendations, and follow-up activities accurately in EPIC. Review and manage telephone encounters, patient messages, test results, medication-related questions, and provider-directed follow-up within EPIC. Coordinate referrals, follow-up appointments, diagnostic testing, and other aspects of patient care as directed. Communicate abnormal or concerning symptoms and clinical findings to the appropriate provider for further evaluation. Monitor and follow up on patient concerns to ensure continuity of care and appropriate resolution. Assist providers with clinical tasks and patient care coordination as needed. Maintain accurate and confidential patient records in accordance with HIPAA and organizational policies. Participate in quality improvement initiatives, staff meetings, training, and ongoing professional development. Demonstrate strong clinical judgment, critical thinking, organization, and prioritization in a fast-paced family medicine environment. Provide exceptional customer service while maintaining professional boundaries and patient confidentiality. Qualifications 1+ year in a triage setting EPIC experience Triage experience with all ages (preferred) Experience Minimum 1 year in a triage setting.
