You've already learned one of the more challenging parts of home health nursing: walking into a new patient's home, completing a comprehensive assessment, navigating OASIS, reconciling medications, educating the family, and developing an accurate picture of what that patient needs. That's specialized experience, and we're looking for RNs who want to put it to work on the weekend. Home Health Care, Inc. is hiring experienced Weekend Start of Care RNs throughout the Northern Greater Twin Cities Metro Area , with opportunities to work every weekend or every other weekend . You'll earn $200 for every Start of Care visit plus mileage reimbursement while helping us admit patients who shouldn't have to wait until Monday for home health care to begin. When someone is ready to return home, getting home health started promptly can help provide the education, clinical oversight, and services they need during an important transition. As the Weekend Start of Care RN, you'll complete the initial assessment, determine the patient's clinical and safety needs, establish the initial plan of care, and communicate your findings to the clinicians who will continue working with the patient. What You'll Be Doing Complete comprehensive Start of Care assessments Perform accurate and timely OASIS documentation Complete medication reconciliation and identify concerns Assess clinical, functional, environmental, and safety needs Develop individualized plans of care Educate patients and caregivers on medications, diagnoses, safety, and home health expectations Identify changes or concerns requiring provider communication Coordinate with physicians and referring providers Collaborate with RN Case Managers, therapists, and other members of the care team Complete thorough documentation that supports a smooth transition to ongoing care We're Looking for an RN Who Already Knows Home Health Start of Care visits require more than strong general nursing skills. You need to understand OASIS, home health documentation, care planning, medication reconciliation, and how to independently assess a patient in their own environment. If you're already comfortable with those responsibilities, we'd rather help you put that experience to work than have it sit unused on your weekends.

What You Bring Required:

Active Minnesota Registered Nurse (RN) license Experience completing Start of Care assessments Strong working knowledge of OASIS Previous home health nursing experience Strong assessment and critical-thinking skills Accurate and timely clinical documentation Confidence working independently in patient homes Valid driver's license and reliable transportation
