A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.
Overview We are seeking a compassionate and dependable Registered Nurse to join our home healthcare team. In this role, you will provide and coordinate high-quality, person-centered nursing services for clients in their homes. You will help clients maintain their health, safety, dignity, and independence while supporting their families and caregivers. This position includes client assessments, care planning, medication management, skilled nursing visits, staff supervision, and coordination with healthcare providers. The ideal candidate is organized, clinically knowledgeable, and passionate about serving older adults and individuals with disabilities, chronic conditions, or complex healthcare needs. Responsibilities Complete initial and ongoing nursing assessments in clients' homes. Develop, implement, review, and update individualized plans of care. Perform skilled nursing visits and monitor changes in clients' health conditions. Monitor vital signs, medications, symptoms, nutrition, mobility, skin integrity, and overall well-being. Provide medication management, medication setup, education, and monitoring according to provider orders. Administer prescribed medications, injections, and other treatments within the RN's scope of practice. Provide wound care, catheter care, ostomy care, diabetic care, and other authorized skilled nursing services. Coordinate care with physicians, pharmacies, case managers, therapists, families, and other members of the care team. Educate clients, families, and caregivers about medications, diagnoses, treatments, safety, and disease management. Complete supervisory visits and provide clinical guidance to LPNs and unlicensed direct-care staff. Ensure caregivers follow each client's care plan, service agreement, and safety requirements. Document assessments, visits, interventions, communications, and changes in condition accurately and promptly. Report significant changes, medication concerns, incidents, and suspected abuse or neglect according to agency policy. Support infection prevention, emergency preparedness, and quality-improvement activities. Maintain client confidentiality and comply with HIPAA, agency policies, and applicable Minnesota home-care requirements. Participate in care conferences, staff training, and case reviews as needed. Qualifications Current, unrestricted Registered Nurse license in the State of Minnesota. Graduate of an accredited nursing program. Current CPR/BLS certification. Previous home care, community nursing, long-term care, disability services, geriatrics, or skilled nursing experience preferred. Strong assessment, medication-management, documentation, and care-coordination skills. Knowledge of infection-control practices, client safety, and person-centered care. Ability to work independently and make sound clinical decisions in a home-care setting. Excellent communication, organization, time-management, and teamwork skills. Comfortable working with older adults and individuals with disabilities, developmental needs, memory-care needs, or chronic health conditions. Ability to maintain accurate electronic and paper documentation. Valid driver's license, reliable transportation, and ability to travel to clients' homes.
