The responsibilities for this role include: Serve as the primary RN Case Manager for an assigned group of hospice patients. Complete comprehensive nursing assessments and develop individualized plans of care. Provide skilled nursing care focused on comfort, dignity, and quality of life. Monitor change in condition and manage pain and other symptoms. Educate and support patients, families, and caregivers throughout the hospice experience Coordinate care with physicians, hospice aides, social workers, chaplains, and other members of the interdisciplinary team Communicate effectively with patients, families, providers, facilities, and referral partners Provide medication education and support appropriate medication management Complete accurate and timely clinical documentation Respond appropriately to changes in patient condition and provide clinical guidance to caregivers Advocate for the patient's goals, wishes, dignity, and comfort Provide support and education to facility staff and caregivers when appropriate Participate in the hospice on-call rotation as required What you'll need: Current MN Registered Nurse License Confident making clinical decisions while knowing when to seek additional support Ability to work independently and manage multiple patients and priorities Valid Driver's License Reliable Transportation Benefits for
