You'll manage a hospice caseload throughout Mankato and Southern Minnesota , working independently in the community while collaborating with physicians and an interdisciplinary hospice team. You're Managing Care. As an RN Case Manager, you're responsible for understanding the bigger clinical picture. You'll assess patients, manage symptoms, monitor disease progression, adjust plans of care, communicate changes, coordinate resources, educate caregivers, and anticipate what your patients may need next. You're the nurse connecting the pieces. That requires clinical confidence, organization, accountability, and the ability to make decisions independently . What You'll Own Manage an assigned hospice patient caseload Complete comprehensive nursing assessments Develop and update individualized plans of care Manage pain, symptoms, medications, wounds, and changing clinical needs Recognize changes in condition and coordinate interventions Educate patients and caregivers about medications, symptoms, disease progression, and comfort measures Communicate directly with physicians and providers Coordinate with hospice aides, social workers, chaplains, facilities, and other members of the interdisciplinary team Support patients and families through end-of-life transitions Maintain accurate and timely documentation Advocate for each patient's comfort, dignity, wishes, and quality of life Bring the Clinical Experience You've Already Built Hospice experience is preferred, but strong RNs come from many backgrounds. We're interested in nurses with experience in Home Health, Palliative Care, Oncology, ICU, Critical Care, Skilled Nursing, Long-Term Care, Assisted Living, Community Nursing, Complex Care, or Care Management . If you're comfortable managing complex patients, making independent decisions, communicating with providers, and taking responsibility for the overall plan of care, you may already have the foundation we're looking for . What We're Looking For Active Minnesota Registered Nurse (RN) license Strong clinical assessment and critical-thinking skills Ability to independently manage a patient caseload Excellent organization and follow-through Strong patient and family education skills Confidence communicating with physicians and other healthcare professionals Ability to work independently while collaborating with a clinical team Valid driver's license and reliable transportation Willingness to travel throughout Mankato and surrounding Southern Minnesota communities Hospice experience preferred, but strong transferable nursing experience is encouraged You've Earned More Than a Task List.
