What You'll Be Doing Complete comprehensive Start of Care nursing assessments Perform accurate and timely OASIS documentation Complete medication reconciliation and identify clinical concerns Assess medical, functional, environmental, and safety needs Develop individualized plans of care based on your findings Educate patients and caregivers on medications, diagnoses, safety, and home health expectations Communicate concerns with physicians and referring providers Coordinate with RN Case Managers, therapists, and the interdisciplinary team Complete thorough documentation to support continuity of care Who We're Looking For This position is designed for an RN who already understands home health and can independently manage the clinical and documentation requirements of a Start of Care.
