Home Health Care, Inc. is looking for experienced RNs who want to put those skills to work every weekend or every other weekend throughout the Greater Twin Cities Metro Area. At $200 per Start of Care plus mileage reimbursement , this is an opportunity to earn additional income doing work you already know rather than picking up another traditional nursing shift. What You'll Be Doing Complete comprehensive Start of Care nursing assessments Perform accurate and timely OASIS documentation Complete medication reconciliation and identify clinical concerns Assess medical, functional, environmental, and safety needs Develop individualized plans of care based on your findings Educate patients and caregivers on medications, diagnoses, safety, and home health expectations Communicate concerns with physicians and referring providers Coordinate with RN Case Managers, therapists, and the interdisciplinary team Complete thorough documentation to support continuity of care Who We're Looking For This position is designed for an RN who already understands home health and can independently manage the clinical and documentation requirements of a Start of Care. We're especially interested in nurses with experience as Home Health Admissions RNs, RN Case Managers, Start of Care Nurses, or experienced field RNs. Required Qualifications Active Minnesota Registered Nurse (RN) license Previous home health nursing experience Experience completing Start of Care assessments Strong working knowledge of OASIS Strong assessment and critical-thinking skills Accurate and timely clinical documentation Confidence working independently in patient homes Excellent patient, caregiver, and provider communication Valid driver's license and reliable transportation Turn Experience You've Already Earned Into Additional Income You don't need another nursing job that requires learning an entirely new system of care.
