Home Health Care, Inc. is hiring a Registered Nurse (RN) Case Manager to serve patients throughout Richfield, Hopkins, Edina, St. Louis Park, and Southwest Minneapolis (55410 and 55416). Find the Problem Behind the Problem Sometimes the answer isn't in the chart. You walk into the home and discover the patient can't read the medication labels. The diabetic diet isn't realistic for what they actually eat. They're falling because the walker doesn't fit through the hallway. The caregiver everyone thought was available is completely overwhelmed. Home health lets you see the full picture. Then you get to use your nursing experience to do something about it. Assess the situation. Educate the patient. Communicate with the physician. Involve therapy. Update the plan of care. Follow up and see whether it worked. Why RNs Take a Look $85,000-$130,000+ annual earning potential $6,000 First-Year Bonus Monday-Friday, 8:00 a.m.-5:00 p.m. Focused Southwest Metro territory One-on-one skilled nursing Ownership of an assigned patient caseload Field-based independence Mileage reimbursement between visits Cell phone reimbursement every paycheck Free MedBridge Education Hub Nursing licensure reimbursement Annual scrub reimbursement Comprehensive benefits PTO and paid holidays 401(k) with employer match Experienced clinical and interdisciplinary support More than 30 years serving Minnesota home health patients Put Your Clinical Judgment to Work As the RN Case Manager, you'll: Manage an assigned home health caseload Complete comprehensive nursing assessments Perform admissions, recertifications, and ongoing skilled visits Develop and update individualized plans of care Complete OASIS assessments and documentation Provide medication management and disease education Manage wounds and other skilled nursing needs Monitor chronic and medically complex conditions Recognize and respond to changes in condition Educate patients and caregivers Communicate directly with physicians Collaborate with PTs, OTs, SLPs, social workers, and other clinicians Help prevent avoidable emergency department visits and rehospitalizations Participate in an on-call and weekend rotation Your Experience Has Taught You What to Look For We're interested in experienced RNs from Home Health, Med/Surg, Acute Care, ICU, Transitional Care, Skilled Nursing, Rehabilitation, Case Management, and other clinically complex settings . You'll need an active Minnesota Registered Nurse (RN) license , three or more years of nursing experience , strong assessment and critical-thinking skills, excellent organization and communication, and a valid driver's license with reliable transportation. Home health, OASIS, and previous case management experience are preferred. Sometimes You Need to See the Home to Understand the Patient A chart can tell you the diagnosis. Labs can tell you what's happening physiologically. But neither can always tell you why the patient keeps struggling once they get home. That's where your experience matters.
