Legacy Home Care and Home Health Care, Inc. are hiring a Floating Visiting Nurse (RN) to provide skilled nursing care throughout the Northern Greater Twin Cities Metro Area . You'll organize your visits, travel throughout your assigned coverage area, walk into each home prepared to assess what's happening, provide the skilled care ordered, educate the patient, communicate concerns, document your visit, and keep moving. As a Floating Visit RN, you'll also support different patients and caseloads based on where nursing coverage is needed most. You have a team behind you without having someone standing over you. One patient may need wound care. Another may have recently discharged from the hospital and need medication reconciliation and education. Another may have a chronic condition that requires skilled assessment and monitoring. You're the RN standing in the home, which means you need to be able to assess, think critically, recognize concerns, and determine when something needs to be communicated or escalated. That's a responsibility, but for a confident nurse, it's also one of the best parts of working in the field. What You'll Do Provide skilled nursing visits throughout the Northern Greater Twin Cities Metro Area Complete comprehensive nursing assessments Perform wound care and other ordered skilled treatments Provide medication management and education Monitor patients following hospitalization and changes in condition Provide chronic disease, safety, and fall-prevention education Recognize changes in condition and communicate concerns appropriately Support admissions and additional skilled visits as needed Provide coverage based on changing patient and clinical needs Collaborate with RN Case Managers, physicians, therapists, social workers, and other care team members Complete accurate and timely EMR and OASIS documentation when applicable Participate in required on-call and weekend rotations You May Already Have the Skills for Home Health We're interested in RNs coming from Home Health, Med/Surg, Acute Care, Transitional Care, Skilled Nursing, Hospice, Wound Care, Rehabilitation, Assisted Living, and Community-Based Nursing . If you're comfortable assessing patients, performing skilled treatments, educating families, recognizing changes in condition, staying organized, and making clinical decisions, many of the skills you've already developed can translate well into the field. Home health experience is preferred, but confidence, clinical judgment, organization, and the ability to work independently matter tremendously. What We're Looking For Active Minnesota Registered Nurse license Strong hands-on nursing and assessment skills Confidence working independently in patient homes Ability to recognize and appropriately respond to changes in condition Excellent patient and caregiver education skills Strong organization, communication, and documentation abilities Valid driver's license and reliable transportation Willingness to travel throughout the Northern Greater Twin Cities Metro Area Participation in on-call and weekend rotations required Home health experience preferred OASIS experience preferred You've Earned the Experience.
