Facilitates coordination of a multidisciplinary plan of care to guide patient progress to optimal care transitions Performs evaluation of patient's care and progress for the episode of care and/or across the continuum of care for the identified population Reviews patient medical records to determine justification of admission, treatment and continued stay criteria Determines and facilitates the appropriate post-hospital discharge destination for a patient Identifies patient needs, capacity and preferences to determine what is required for a smooth and safe transition to patient's discharge destination Actively involves patient, or individual acting on patient's behalf, in planning for discharge Job Requirements Required Diploma Of Nursing Graduate of accredited professional nursing education program
Registered Nurse:
MN Preferred Bachelors Degree in a healthcare field Experience as a Discharge Coordination and/or in Case Management
