Your ability to recognize subtle changes still matters. Your experience with medications, wounds, symptoms, complex disease processes, and patient education still matters. What's different is the goal. Instead of asking, "What else can we do to treat the disease?" , you're helping determine, "What can we do to make today better for this patient?" You'll manage complex symptoms, coordinate care, educate families, anticipate needs, and become a trusted clinical resource throughout each patient's hospice journey. What You'll Own Manage an assigned hospice patient caseload Complete comprehensive nursing assessments Develop and update individualized plans of care Manage pain, symptoms, medications, wounds, and changing clinical needs Recognize changes in condition and coordinate appropriate interventions Educate patients and caregivers about medications, symptoms, disease progression, and comfort measures Communicate with physicians and other providers Coordinate with social workers, chaplains, hospice aides, facility teams, and other members of the interdisciplinary team Support patients and families through end-of-life transitions Maintain accurate and timely clinical documentation Advocate for each patient's comfort, dignity, wishes, and quality of life You May Already Have the Skills Hospice Needs You don't need to have spent your entire career in hospice. We're interested in experienced RNs from Home Health, Palliative Care, Oncology, ICU, Critical Care, Skilled Nursing, Long-Term Care, Assisted Living, Community Nursing, Complex Care, and Care Management . If you can confidently assess a complex patient, communicate with physicians, educate a worried family, manage changing clinical needs, and take ownership of a caseload, your experience may translate exceptionally well to hospice . What We're Looking For Active Minnesota Registered Nurse (RN) license Strong clinical assessment and critical-thinking skills Ability to independently manage a patient caseload Excellent organization and follow-through Strong patient and family education skills Confidence coordinating with physicians and other healthcare professionals Ability to work independently while collaborating with an interdisciplinary team Valid driver's license and reliable transportation Willingness to travel throughout Mankato and surrounding Southern Minnesota communities Hospice experience preferred, but strong transferable nursing experience is encouraged Your Next Chapter Doesn't Have to Look Like Your Last One.
