You'll assess medically complex patients, manage wounds and medications, recognize changes in condition, complete OASIS documentation, communicate with physicians, and coordinate the overall plan of care. Home You'll:
Manage an assigned home health caseload Complete comprehensive nursing assessments Perform admissions, recertifications, and skilled visits Develop and update individualized plans of care Complete OASIS assessments and documentation Provide medication management and disease education Manage wounds and other skilled nursing needs Monitor complex and chronic conditions Recognize and respond to changes in condition Educate patients and caregivers Communicate directly with physicians Collaborate with PTs, OTs, SLPs, social workers, and other clinicians Help prevent avoidable emergency department visits and rehospitalizations Participate in an on-call and weekend rotation Your Experience Can Translate We're interested in experienced nurses from Home Health, Med/Surg, Acute Care, ICU, Transitional Care, Skilled Nursing, Rehabilitation, Case Management, and other clinically complex settings . If you've learned how to assess quickly, prioritize competing needs, educate families, recognize subtle changes, and make independent clinical decisions, you already have skills that matter tremendously in home health. You'll need an active Minnesota Registered Nurse (RN) license , three or more years of nursing experience , strong clinical judgment and organization, and a valid driver's license with reliable transportation. Home health, OASIS, and case management experience are preferred. Trade the Nursing Station for Something Different Your office changes throughout the day.
