A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

On-Call Rotation Compensation:
$95,000 - $130,000 annually plus incentive visit opportunities and $6,000 in first-year bonuses

Complete OASIS assessments and individualized plans of care. Deliver one-on-one skilled nursing visits and patient education. Collaborate closely with physicians and interdisciplinary team members. Support local teams during staffing needs, growth, and coverage. Maintain accurate and timely clinical documentation. What We're Looking For Active Minnesota Registered Nurse (RN) license. Strong clinical assessment and critical thinking skills. Comfortable adapting to different territories and patient populations. Home health experience preferred, but not required. Reliable transportation and willingness to travel throughout the metro.
