Role:

Provide primary and acute care to patients of all ages, with a focus on family practice Conduct thorough patient assessments, develop treatment plans, and prescribe medications as needed Collaborate with our experienced physician team to ensure comprehensive patient care Stay up-to-date with the latest medical research, technologies, and best practices to provide exceptional care Foster a positive and respectful team atmosphere by working collaboratively with healthcare professionals

Responsibilities:

Conduct medical exams, take medical histories, and diagnose and treat common illnesses and injuries Order and interpret laboratory tests and imaging studies as needed Develop and implement individualized treatment plans, taking into account patient needs, preferences, and health status Prescribe medications and therapies as needed, and monitor patient response to treatment Educate patients and families about healthy habits, disease prevention, and wellness strategies Stay current with changes in medical policies, procedures, and regulations Preferred Qualifications Two (2) or more years of Nurse Practitioner (NP) experience Experience in Family Practice, Adult, Skilled Nursing, Long-Term Care, Geriatrics, Home-Based Primary Care, or Acute Care Experience with PointClickCare or other Long-Term Care EHR systems Knowledge of CMS Skilled Nursing Facility regulations Familiarity with value-based care models, ACOs, I-SNPs, and geriatric prescribing principles

Compensation Competitive Total Compensation:
