Primary Responsibilities:
Perform comprehensive annual or post-discharge home health risk assessments in patient residences Review and document prior and current medical history, conditions, and medication use Educate patients regarding their health, medications, and illness management Identify care gaps and recommend appropriate resolution steps Visits average 45 minutes; no treatment planning or prescribing required
Preferred Skills and Experience:
Active Missouri Nurse Practitioner license in good standing Board Certification (AANP or ANCC) Current BLS/CPR certification MSN earned from an accredited nursing program Minimum of 6 months current nurse practitioner experience preferred Home health experience a plus This position offers compensation of $80-$90 per evaluation, along with a mileage stipend and administrative time.
