Job Overview Life Care Center of Grandview is seeking a dedicated and highly skilled MDS Coordinator (Registered Nurse) to join our healthcare team. The ideal candidate will possess a comprehensive understanding of the Minimum Data Set (MDS) process, long-term care regulations, and clinical documentation standards. As an MDS Coordinator, you will be responsible for coordinating the assessment, documentation, and coding processes to ensure compliance with federal and state regulations, including Medicare and Medicaid requirements. Your expertise will support optimal patient care, accurate reimbursement, and quality improvement initiatives across our facility. Responsibilities Lead the comprehensive assessment process by coordinating with nursing staff, physicians, and other healthcare professionals to gather accurate clinical data for MDS completion. Ensure timely and accurate completion of all MDS assessments in accordance with CMS (Centers for Medicare and Medicaid Services) guidelines and long-term care regulations. Review medical records, clinical documentation, and coding to verify accuracy and completeness, adhering to clinical documentation improvement standards. Utilize EMR (Electronic Medical Records) systems such as Epic, Cerner, or Athenahealth to document assessments and ensure proper data entry. Collaborate with interdisciplinary teams on discharge planning, care planning, and case management to optimize patient outcomes and regulatory compliance. Maintain knowledge of healthcare policies including

HIPAA, HEDIS

(Healthcare Effectiveness Data and Information Set), NCQA standards, and CMS regulations related to long-term care and Medicare reimbursement. Conduct ongoing staff education on documentation standards, ICD-10/ICD-9/ICD coding, CPT coding, DRG (Diagnosis-Related Group) assignment, and utilization management practices. Monitor changes in health regulation policies at the federal and state levels to ensure ongoing compliance across all documentation processes. Support quality assurance initiatives by reviewing medical records for accuracy in medical terminology, anatomy knowledge, physiology details, and clinical documentation standards. Assist in audits related to health information management and participate in continuous process improvements within the facility's documentation workflows. Qualifications Registered Nurse (RN) licensure with current state licensure required; additional certifications in case management or health information management are preferred. Proven experience in long-term care facilities or skilled nursing environments with familiarity in MDS coordination or clinical documentation review. Strong knowledge of Medicare regulations, CMS guidelines, HEDIS measures, NCQA standards, and long-term care regulations. Experience working with EMR/EHR systems such as Epic, Cerner, eClinicalWorks, or Athenahealth is essential. Proficiency in medical coding including

ICD-10/ICD-9

coding systems along with CPT coding for procedures. Familiarity with hospital or inpatient experience as well as outpatient settings enhances effectiveness in this role. Excellent organizational skills with attention to detail; ability to manage multiple assessments simultaneously while meeting deadlines. Strong communication skills for effective collaboration across multidisciplinary teams including nursing staff, physicians, case managers, and administrative personnel. Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times.
