Nurse Practitioners with experience in home health are invited to apply for a per diem opportunity focused on conducting risk assessments for Medicare Advantage plan members. This role centers on evaluating patients in their homes across Boone County, MO, including Columbia and surrounding areas such as Rocheport, McBaine, and Ashland. Assessments are scheduled individually and typically last about 45 minutes. Shifts can take place any day of the week, and practitioners have the flexibility to set their own schedules, with a minimum commitment required on a weekly or bi-weekly basis.

Qualifications:

Active Nurse Practitioner license in Missouri Valid CPR certification (BLS) Board certification through AANP or ANCC 6 months or more of current experience as a Nurse Practitioner preferred Master of Science in Nursing (MSN) from an accredited institution

Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members. Review prior and current medical and medication histories. Educate patients on their medical conditions and prescribed medications. Identify potential gaps in care and offer recommendations to address them.
