Key Responsibilities:

Complete annual home health risk assessments during patient home visits (approximately 45 minutes per appointment) Review and assess patients health history, current conditions, and medication regimens Provide patient education on health conditions, prevention, and medication management Detect potential gaps in care and recommend solutions to improve health outcomes No medication prescribing or treatment planning required

Qualifications:

Active Nurse Practitioner license in Missouri MSN from an accredited School of Nursing Certified by AANP or ANCC Current, valid CPR certification (BLS) Minimum of 6 months nurse practitioner experience preferred You'll receive compensation of $80-$90 per evaluation, with a mileage stipend and additional consideration for administrative time.
