Nurse Practitioners with a passion for patient-centered care are sought to perform comprehensive home health risk assessments for Medicare Advantage plan members in Barry County, MO. In this per diem opportunity, youll meet individuals in their homes in areas such as Monett, Savannah, Country Club, and Fillmore, conducting evaluations that support preventive care and care gap closure.

Flexible shifts available Monday through Sunday Set your own schedule Options for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

Conduct annual and post-discharge home health risk assessments, averaging 45 minutes per visit Review prior and current medical conditions, medical history, and medication regimens Provide education to members regarding their medical conditions and medications Identify gaps in care, offering recommendations to address these gaps and ensure optimal health outcomes

This role does not require prescribing medication or developing treatment plans

Qualifications:

Active Nurse Practitioner license in Missouri (MO) Current CPR certification (BLS) Board Certification via AANP or

ANCC MSN

degree from an accredited nursing program A minimum of 6 months recent experience as a Nurse Practitioner is preferred This per diem engagement provides excellent compensation, offering $80-$90 per evaluation, along with mileage reimbursement and compensation for administrative time.
