Key Responsibilities:

Perform in-home annual risk assessments for Medicare Advantage plan members Review and evaluate prior/current medical history, medical conditions, and medication history Provide clear education to patients regarding their health conditions and prescribed medications Identify care gaps and offer actionable recommendations for closing them No requirements for developing treatment plans or prescribing medications

Qualifications:

Nurse Practitioner license (MO) in good standing Board certification through AANP or ANCC Current CPR certification (BLS) MSN earned from an accredited nursing school Minimum of six months recent Nurse Practitioner experience is preferred
