Job Description

Delivering high-quality, patient-centered care in the home is a rewarding way to make a direct impact on individual health outcomes. As a Home Health Nurse Practitioner, you will travel within Adair County, meeting with Medicare Advantage members to conduct comprehensive risk assessments. Your visits will help identify care gaps and provide crucial educationno treatment plan or prescription management required.

Flexible shifts available Monday through Sunday Self-scheduling tailor your workload to fit your life Choose minimum weekly or bi-weekly hourly commitments
Responsibilities:
Complete annual home health risk assessments for Medicare Advantage plan members throughout Adair County, including areas like Kirksville, Greentop, and Novinger Review prior and current medical history, chronic conditions, and medication use Educate patients on their diagnoses, treatments, and medication management Identify gaps in care, document findings, and recommend follow-up actions Focus exclusively on assessment and educationno need to create or manage treatment plans or prescribe medications
Qualifications:
Current Nurse Practitioner license in Missouri Board Certification through AANP or ANCC Graduation from an accredited MSN program Active CPR certification (BLS) 6+ months of NP experience preferred Youll receive $80$90 for each evaluation, along with a mileage stipend and compensation for administrative work associated with visits.
