Schedule:
Flexible shifts available Monday through Sunday Self-scheduling to fit your availability Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
Perform comprehensive home health risk assessments for Medicare Advantage plan members Assess prior and current medical history, evaluate chronic conditions, and review medication regimens Deliver patient education on health management and medication use Identify and address any care gaps with clear follow-up recommendations No creation of formal treatment plans or prescription writing needed
Qualifications:
Valid Nurse Practitioner license in MO Active CPR certification (BLS required) Board certification through AANP or ANCC Minimum of 6 months recent nurse practitioner experience preferred MSN degree from an accredited nursing program Youll be conducting visits in locations throughout Cedar County, including El Dorado Springs, Stockton, and Jerico Springs. Each evaluation takes approximately 45 minutes, with diversity in visit types including annual health assessments and post-discharge follow-ups. Expect to receive $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
