Responsibilities:

Conduct home health risk assessments for Medicare Advantage members throughout Nodaway County, including Maryville, Burlington Junction, and Hopkins Each risk assessment averages 45 minutes, including annual wellness assessments and post-discharge visits Review and evaluate patients current and prior medical and medication histories Educate members on medical conditions and proper medication usage Identify potential gaps in care and offer effective recommendations No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Missouri Board Certification through AANP or ANCC Current CPR certification (BLS) Master of Science in Nursing from an accredited nursing program At least 6 months of current experience as a Nurse Practitioner preferred This per diem role provides a competitive rate of $80-$90 per completed evaluation, plus a mileage stipend and administrative time compensation.
