Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Review medical history, assess current conditions, and evaluate medication use Provide patient education related to existing medical issues and medication management Identify any gaps in care and recommend steps to address these concerns No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner License in Missouri Current CPR certification (BLS) Board Certification through AANP or ANCC 6 months or more of current nurse practitioner experience preferred Master of Science in Nursing (MSN) from an accredited program As part of this role, youll be compensated at $80-$90 per evaluation, in addition to a mileage stipend and administrative time reimbursement.
