Citizens Memorial Hospital (CMH) is recruiting a Community Resource Specialist to work with our amazing team in Stockton and Greenfield — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.

Position Details Status:

Full-time Work Shift:

Days Hours:

Monday-Friday, 8am-5pm

What Your Day Looks Like:

In this role, you will support patients navigating the healthcare system, removing barriers to care, and referring to community resources to address social determinants to health for patient safety. As a Community Resource Specialist, you will be: Providing direct support to patients and families by addressing socio-economic factors impacting health, both in-person and by phone. Receiving referrals for specialty social service support for clinic patients and providing timely follow-up. Performing social needs screening and advising of applicable resources and solutions available. Documenting patient-centered plans of care. Collaborating and coordinating care with an interdisciplinary care team and advocating for patient safety in complex and challenging situations.

Qualifications Education:

High school diploma

Skills:

Written and oral communication skills, time management, detail-oriented

Licensure/Certification:

Willingness to complete CHW certification within 6mo of hire ;

Preferred:

Current CHW Certification Physical Requirements:
