Job Overview We are seeking a dedicated and compassionate Registered Nurse (RN) to join our long-term care facility on a full-time basis, working day shifts. The RN will collaborate closely with interdisciplinary teams to deliver comprehensive, person-centered care while maintaining compliance with healthcare standards and regulations. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews, to develop individualized care plans. Administer medications and injections accurately. Monitor residents' conditions continuously, utilizing EMR/EHR systems for documentation and ensuring timely updates on patient status. Provide wound care, including suturing and ostomy management, adhering strictly to aseptic techniques to prevent infections. Collaborate with physicians and specialists in areas such as cardiology, ophthalmology, infectious disease care, and hospice & palliative medicine for comprehensive treatment planning. Assist with diagnostic evaluations like laboratory specimen collection and processing, ensuring proper handling and documentation. Educate residents and their families on health management strategies, including pain management, memory care techniques, and discharge planning. Ensure compliance with HIPAA regulations and maintain confidentiality of all patient records in accordance with legal standards. Qualifications Valid RN license with current registration in the state or jurisdiction of employment. Proven experience in long-term care settings or nursing homes; experience working with individuals with disabilities or dementia is highly desirable. Knowledge of EMR/EHR systems such as PCC; familiarity with ICD coding (ICD-9/ICD-10) is preferred. Strong clinical skills including IV insertion, medication administration, tube feeding management, and basic life support certification. Ability to perform physical examinations, vital signs monitoring, and diagnostic evaluation competently. Excellent communication skills coupled with a compassionate approach to patient care; ability to work effectively within multidisciplinary teams.
