Job Description

CCoordinates the daily operations of the Hospitalist Program to ensure efficient patient care, provider communication, and continuity of services. Responsibilities include coordinating physician schedules; serving as the liaison between hospitalists, referring primary care providers, specialists, hospital staff, patients, and families; facilitating communication regarding patient admissions through the Emergency Department; participating in and coordinating discharge planning activities with Case Management to support timely and appropriate transitions of care; scheduling post discharge follow up appointments; coordinating follow up of pending diagnostic tests and ensuring appropriate communication after discharge; ensuring discharge summaries are forwarded to the appropriate providers; collecting physician billing documentation; screening calls; assisting with medical records and billing processes; coordinating physician access to patients and families; supporting physician performance on quality metrics and regulatory measures through data tracking, communication, and follow up; and promoting effective interdisciplinary collaboration.
