PM Key Qualifications:

Current RN license (in good standing) for the state of practice At least 1 year of recent Home Health nursing experience; field-based knowledge preferred Proficiency with Home Care Home Base (HCHB) electronic documentation Demonstrated experience with OASIS assessment tools Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

Complete comprehensive OASIS assessments and create individualized care plans for each patient Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management Monitor patient progress, identifying and reporting changes in condition promptly to physicians and the care team Educate patients and their families on treatment protocols, medications, and preventive health practices Accurately document care using Home Care Home Base (HCHB) Collaborate with a multidisciplinary teamincluding therapists and home health aidesto coordinate high-quality care Participate in regular staff meetings, case conferences, and ongoing professional development Adhere strictly to all regulatory and agency guidelines for documentation and delivery of care
